This form will be used by Geashill National School for the sole purpose of collecting declarations from parents stating that their children have not an infectious disease and that they have followed all medical and public health guidance with respect to being out of school. The form will be used by parents when their children are returning to school after any absence and/or school closures.
All information supplied on this form is for the Data Controller of Geashill National School i.e. the Board of Management. It will be held in the school in accordance with the terms and conditions of GDPR as set out in the Data Protection Act 2018 and the school’s own Data Protection Policy.
